Request To Onboard a UCSF Service Provider
UCSF has partnered with Workforce Logiq to onboard Service Providers and their Resources. Requirements for onboarding has been predetermined by UCSF HR as outlined in the standard MSA. Depending on the services to be provided and the level of access to UCSF premises or property, physically or virtually, the onboarding process may include a background screen, health screen, substance abuse screen and the completion of learning modules, required documentation and the issuance of a security badge.
Step 1 - Fill out Request Form

Step 2 - Email completed form
Email completed form to firstname.lastname@example.org
Step 3 - Confirmation
Workforce Logiq will contact you if additional information is required or when onboarding has been completed. For questions about your request please email email@example.com

How will I be billed for the cost of the onboarding?
The department cost center will be billed by Workforce Logiq after the onboarding has been completed.

The department cost center will be billed by UCSF Security after the badge has been issued.
How will the service provider pay for the background fees?
The service provider will be billed by Workforce Logiq after the onboarding has been completed.
What criteria must be met to onboard my service provider?
Requirements have been predetermined by UCSF hr. They are based on level of access a resource will be granted as per UCSF HR.
What do you need from me?
You may be asked to qualify information that was provide to Workforce Logiq by you or the service provider.
How do I off board or terminate a resource's access?
Simply send an email to Onboarding@workforcelogiq.com
When does my resource's onboarding expire or does it?
The project end date is what governs the onboarding term. Unless they are terminated before the project end date.
